Legacy Healthcare LLC is seeking a Hospital Liaison for the greater Quad Cities region. This role requires a nursing degree (RN, LPN, or ARNP) and at least one year of nursing experience, preferably in sub-acute care. The Hospital Liaison will facilitate transitions for new residents from acute to post-acute care, manage hospital referrals, and ensure quality patient care.
