New Haven Center for Nursing & Rehab in New Haven, CT has an opening for a   Part-time or PRN Physical Therapist .  New graduates are welcome to apply!

• Located at 181 Clifton St, New Haven, CT 06513

• 150 bed SNF

Responsibilities:

· Evaluate, plan and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing

· Assure all treatment is delivered in accordance with an established plan of care

· Provide clinical support and instruct patients, families, and caregivers

· Monitor patient response to treatment intervention

· Complete required forms and documentation in accordance with company policy and state/federal regulations
