Geriatrics Nurse Practitioner

This role is designed for an experienced Nurse Practitioner (NP) who is comfortable practicing in skilled nursing and assisted living environments while collaborating closely with physicians and interdisciplinary care teams.

The position focuses on managing a panel of medically complex geriatric patients across Assisted Living (AL), Independent Living (IL), and Skilled Nursing Facilities (SNFs). The goal is to deliver high-quality, relationship-based primary and post-acute care while reducing avoidable hospitalizations and improving patient outcomes.

This is a full-time clinical role with a strong emphasis on autonomy, mentorship, and long-term professional growth in geriatric medicine.
