Patient Intake Coordinator

ProMedix Health is a Medicare-focused Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) company serving physician practices and health system partners nationwide. We partner with healthcare providers to deliver proactive, technology-enabled care to Medicare patients living with chronic conditions, improving outcomes while reducing the burden on both patients and practices.

We are seeking a motivated, compassionate, and results-driven Patient Onboarding Specialist to join our growing patient outreach team. In this role, you will engage directly with Medicare beneficiaries (65+) to educate them about Medicare-covered health programs, guide them through the enrollment process, and provide ongoing support to ensure a positive patient experience.

A genuine understanding of chronic conditions such as hypertension and diabetes, strong communication skills, and the ability to build trust with patients are essential for success in this role. The ideal candidate is comfortable working with modern technology, including AI-powered platforms, scheduling systems, CRM software, and telehealth tools.

POSITION OVERVIEW

This role focuses on inbound and outbound patient outreach and requires a compassionate, patient centered approach when educating Medicare beneficiaries and guiding them through eligibility verification, enrollment, scheduling, and onboarding processes.

The ideal candidate thrives in a fast paced environment, is comfortable working toward performance goals, and possesses excellent communication, organizational, and multitasking skills. Candidates should be highly skilled at managing patient schedules, coordinating virtual care appointments, and supporting patients through telehealth visits.

KEY RESPONSIBILITIES:

Patient Outreach, Enrollment & Engagement

• Conduct inbound and outbound calls to Medicare beneficiaries while meeting or exceeding company KPIs for call volume, appointments, and enrollments.
• Educate patients on the benefits of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other virtual care services.
• Build trust and rapport using a compassionate and consultative approach.
• Address questions, concerns, and objections while guiding patients through enrollment.
• Conduct eligibility verification, obtain consent, and complete onboarding documentation.
• Maintain ongoing communication and follow-up with prospective and enrolled patients.
• Meet or exceed daily, weekly, and monthly performance goals.

Scheduling & Appointment Coordination

• Manage and maintain patient appointment schedules to ensure efficient provider utilization and patient engagement.
• Confirm upcoming appointments with patients via phone, text, email, or other approved communication channels.
• Reschedule missed or canceled appointments and maintain accurate scheduling records.
• Coordinate provider calendars and ensure patients receive timely appointment reminders.
• Monitor patient attendance and conduct appointment follow-up activities when necessary.

Virtual Visit Support

• Assist patients with connecting to and navigating virtual telehealth appointments.
• Provide technical support before and during virtual visits to ensure a smooth patient experience.
• Coordinate with providers and patients to resolve issues impacting scheduled virtual appointments.
• Help patients understand telehealth technologies and answer basic questions regarding virtual visit participation.
• Facilitate communication between patients and providers before, during, and after virtual visits.

Technology, AI & Systems

• Utilize CRM systems, scheduling platforms, call center software, communication tools, and other healthcare technologies.
• Use approved AI platforms and AI-assisted productivity tools to improve efficiency and workflow management.
• Quickly learn and adapt to new software, systems, processes, and digital technologies.
• Maintain accuracy when transitioning between multiple systems and platforms.
• Follow company guidelines regarding the secure and appropriate use of technology and patient information.

Documentation & Compliance

• Accurately document all patient interactions, scheduling activities, enrollment outcomes, and follow-up actions.
• Maintain compliance with HIPAA, CMS regulations, and company policies.
• Submit activity reports and participate in quality assurance initiatives.
• Ensure all patient information is handled securely and confidentially.
• Maintain detailed and accurate patient records across multiple systems.

Required Qualifications

• In Person Position 8 am to 5 pm
• 1 to 3 years of experience in telesales, healthcare sales, patient enrollment, patient scheduling, appointment coordination, or customer service.
• Strong communication and relationship building skills.
• Experience managing appointment schedules and coordinating patient interactions.
• Comfortable assisting patients with virtual care technology and telehealth platforms.
• Basic knowledge of medical terminology, Medicare programs, healthcare services, or chronic care management.
• Must have experience using CRM systems, scheduling software, Microsoft Office applications, and communication platforms.
• Comfort using AI powered tools and technology enabled workflows.
• Strong organizational, multitasking, and time management skills.
• Ability to accept coaching, adapt to change, and learn new systems quickly.

Preferred

• Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), or telehealth programs.
• Background in medical telesales, patient outreach, healthcare enrollment, insurance sales, or provider relations.
• Experience confirming patient appointments and managing provider schedules.
• Experience supporting virtual healthcare visits and telehealth operations.
• Experience working with Medicare beneficiaries.
• Experience supporting U.S. based healthcare organizations.
• Experience using AI assisted productivity tools, CRM systems, scheduling platforms, and healthcare technologies.
• Bilingual skills (Spanish or other languages) are a plus.
