Discharge Planner

These roles provide essential assistance to healthcare professionals in delivering patient care. Spends majority of time leading the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills normally acquired through advanced education (typically University).

MINIMUM REQUIREMENTS:

Education: Bachelor's degree required.

Experience: None.

Licensure: None.

PHYSICAL DEMANDS:

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.

The starting base rate for this role is $21.10 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
