Family Medicine APP Full-time-Float both FM/IM Clinic Locations
Float Salt Lake Valley- Intermountain Health Covering both Family and Internal Medicine Clinics in the Salt Lake Valley, 25 Buildings, covering long term leaves, and current provider templates.

Minimum Qualifications:

• Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
• Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
• Eligible for State Controlled Substance License II through V.
• Eligible for Federal DEA Certificate II through V.
• BLS Certification for Healthcare Providers.
• Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

Preferred Qualifications:

• Two (2) or more years of experience preferred.
• Master's degree. Education must be obtained from an accredited institution. Degree will be verified.
