Job Summary:
Long Term Care Nurse Practitioner position in Beaver Dam, Wisconsin. This role involves providing nursing care in a nursing home facility that specializes in caring for elderly patients with conditions such as dementia, frailty, and rehabilitation needs. The position includes responsibilities such as charting, assessments, charge duties, medication administration, and clinical skills including IV therapy, catheterization, and ostomy management.

Job Details:

• Location: Beaver Dam, WI
• Schedule: 12-hour days, every other weekend and holiday
• Required Qualifications: BLS (American Heart Association); minimum of 1 year of experience in elderly care; Wisconsin nursing license; no minimum years of experience required if qualified
• Preferred Qualifications: Elderly care experience; IV therapy skills; adult geriatrics knowledge
• Schedule: Block schedule with 8 nurses, 4 during day/pm shift, 2 nurses, and 3-4 aides at night; minimum of 36 hours per week
• Float: Float rotation may be required
• Additional Information: Candidates must note if they have worked for the organization previously; if so, their employment must have ended at least 6 months prior to current submission. All clinicians are expected to clock in and out at the facility.
• Other: On-site pharmacy with medication dispensing system; interdisciplinary team includes therapy, social workers, activities, dietary, housekeeping; RT on-site 24/7; physician availability after hours; unit-specific orientation varies based on skills.
