In this leadership role, you will oversee the daily clinical operations of our home health agency, ensuring high-quality patient care, compliance with all regulatory standards, and exceptional team support. Responsibilities

• Assess patient needs and develop individualized care plans.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on health management and resources.
• Facilitate discharge planning and transitions of care.
• Maintain accurate medical records and documentation.
• Ensure compliance with HIPAA regulations and standards of practice.
• Advocate for patients' needs within the healthcare system

Qualifications

• Active RN license in the state of practice.
• Proven experience in case management or similar roles.
• Strong assessment and critical thinking skills.
• Knowledge of managed care principles and discharge planning processes.
• Excellent communication and interpersonal skills.
• Familiarity with medical terminology and documentation standards.
• Ability to work collaboratively in a team environment.
