RN Nurse Assessor - Home Care

Location: Oneonta, NY area
Job Type: Full-time
Salary: $75,000 - $95,000 DOE
Schedule: Monday to Friday, no on-call
Travel: Field-based with regional travel

At Home Care Partners is seeking a full-time RN Nurse Assessor to support our growing home care team in the Oneonta, NY area.

This is a field-based role focused on start-of-care assessments, reassessments, and helping open new home care cases. The nurse will travel to patients' homes throughout the counties surrounding Oneonta, with occasional travel to areas such as Syracuse and Binghamton as needed.

Home care experience is preferred, but not required. We are willing to train the right RN who is organized, reliable, comfortable working independently, and has strong clinical judgment.

Responsibilities

• Complete start-of-care assessments for new home care patients
• Conduct reassessments for existing patients as needed
• Help open new home care cases by assessing patient needs
• Travel throughout the Oneonta region and surrounding counties
• Occasionally travel to Syracuse, Binghamton, and other areas as needed
• Complete accurate and timely clinical documentation
• Communicate patient updates to the Director of Patient Services
• Work with the clinical and operations team to support case openings
• Follow agency policies, clinical standards, and home care regulations
• Complete limited supervisory visits when needed

Requirements

• Active New York State RN license required
• Valid driver's license required
• Comfortable with regular regional travel
• Strong assessment and documentation skills
• Able to work independently in the field
• Professional, reliable, and organized
• Home care experience preferred, but not required

Schedule

• Full-time
• Monday to Friday
• No on-call requirement
• Field-based travel required

Compensation and Benefits

• $75,000 - $95,000 per year, depending on experience
