Join a dynamic healthcare team as an RN Care Manager, providing comprehensive coordinated care for adult patients across the Midwest region. This role involves developing, monitoring, and evaluating interdisciplinary care plans to ensure optimal patient outcomes.

Key Responsibilities:

• Conduct thorough needs assessments and identify appropriate care options and services.
• Communicate effectively with patients, families, and interdisciplinary teams to support care coordination.
• Manage patient data related to case management, care planning, and quality improvement analysis.
• Monitor care delivery through patient rounds, documentation, and progress tracking.
• Perform utilization reviews and assist nursing staff with patient care as needed.
• Support rehabilitation training and education for staff when required.
• Ensure compliance with healthcare regulations, including HIPAA privacy and security standards.
• Participate in process improvement initiatives and provide leadership on complex projects.

Education and Experience:

• Completion of an accredited nursing program.
• Minimum of one year experience as a registered nurse.
• Preferred: Bachelor of Science in Nursing (BSN) and care management experience.

Licensure:

• Current RN licensure valid in the applicable state.
