Crossroads Health is seeking a compassionate Patient Navigator-Behavioral Health to be the first contact for clients and families seeking behavioral health and primary integrated services. The role focuses on trauma-informed screening, intake documentation, and coordinating care in collaboration with a multidisciplinary team. Ideal candidates hold an Associate’s degree in human services with a preferred Bachelor’s in social work, Ohio licensure (LPC/LSW/MFT) preferred, and QMHS eligibility #J-18808-Ljbffr Crossroads Health
