This role is ideal for a professional who is passionate about supporting the mental, emotional, and social well-being of nursing home residents and their families. Responsibilities Conduct psychosocial assessments to identify the needs of residents and their families. Provide counseling and emotional support to residents and families regarding adjustment to nursing home life. Develop and implement care plans in collaboration with healthcare team members. Advocate for resident needs, rights, and preferences. Coordinate services and resources for residents, including community resources and support groups. Facilitate resident and family meetings and provide guidance on healthcare, end-of-life planning, and other related topics. Document and maintain accurate records of all interactions and services provided. Assist in discharge planning and post-discharge follow-up care. Qualifications Master's degree in Social Work (MSW) from an accredited program. Active Social Worker license (LCSW or equivalent) in the state of NY. Previous experience in a healthcare or nursing home setting preferred. Strong interpersonal, communication, and organizational skills. Ability to work effectively with a multidisciplinary team and handle sensitive situations with compassion. #J-18808-Ljbffr
