RN Case Manager - Primary Care - Clinic - Okmulgee

The purpose of this position is to facilitate patient admissions/discharges and health benefits coordination to patients of the Muscogee (Creek) Nation Medical Center by providing patient case management.

Minimum Qualifications

Education A Bachelor's Degree in Nursing (BSN) received from an accredited school of professional nursing is preferred. Candidates that have completed an Associate's Degree or other program of at least two (2) years in nursing or a related field may also be considered.

Experience A minimum of one (1) year related experience in a relevant job position.

Licenses & Certification - Incumbent is required to possess current licensure by the State of Oklahoma or National Council of State Boards of Nursing for Registered Nurse and current CPR.

Knowledge & Skills

• Knowledge of all aspects of case management for hospital inpatients.
• Knowledge of acceptable standards of nursing practices, principles and theories.
• Knowledge of IHS and MCNDH policies and procedures as they relate to patient case management.
• Knowledge of the Privacy Act and other related regulations with regards to confidentiality and the release of medical information.
• Knowledge of patient registration procedures and health benefits coordination.
• Ability to communicate and interact effectively with patients and their families, staff members, other MCNDH employees, tribal officials and the public both orally and in writing.
• Knowledge of patient teaching/counseling techniques.
• Knowledge of regulations regarding pre-certification, etc., of various insurance companies.
• Ability to communicate and accept varied lifestyles, healing methods and practices.
• Knowledge and skill to utilize a PC in a Windows environment and to operate other necessary office equipment.
• Ability to communicate courteously and effectively with MCNDH administrative and medical staff, patients and their families, Tribal and Non-Tribal health care professionals and the general public both orally and in writing.

Work Environment

Work is performed in an office, clinical and hospital setting. Risk of exposure to hostile and emotionally disturbed patients, families and visitors does exist. There is a moderate risk of exposure to contagious disease. Incumbent must take precautions as required. Occasional travel may be required. Incumbent must be insurable to drive a GSA vehicle.

Physical Demands

The work requires the demands of normal office work and dealing with patients in a hospital and clinic setting. Occasional driving to other MCNDH facilities and to attend education/training programs/seminars may be required.

Essential Functions

Satisfactory job performance will be determined by successful execution of the following:

1. Develops a plan of care for individual patients by interviewing patient and family members, reviewing patient's health record and determining the kind of health and physical care required.
2. Observe and assesses patient's clinical condition, recognizes, identifies, and interprets serious situations and immediately decides proper action. g. inform the physician and take pre-planned measures.
3. Responsible for conducting diagnostic chart reviews of each patient received from the ambulatory clinics.
4. Consult with the members of the provider team as necessary to facilitate correct decision-making regarding each case.
5. Arrange for consultations with patients and family members and provide them individual guidance and education regarding admittance, discharge and disease management and education.
6. Facilitate patient and family understanding of the policies and procedures of the health system and coordinate the flow of patients within the healthcare process.
7. Prepare treatment and discharge plans for individual patients where indicated.
8. Provide utilization reviews to ensure that services available within the MCNDH are utilized whenever possible.
9. Consult with the referring physician if services are not available to prepare alternate treatment plans based on patient history.
10. Documentation of specific health care requirements for patients. Plans, provides and documents teaching of patients and family members in preventive, curative and rehabilitative measures.
11. Records observations, nursing interventions, therapeutic measures administered and status of coordinated activities between nursing and other professional disciplines.
12. Evaluates the patient status, intervention, patient response and updates the plan of care.
13. Attends committee meetings as required.
14. Knowledge of federal, tribal and state regulations regarding healthcare facilities.
15. Participates in the organization's Continuous Quality Improvement (CQI) program.
16. Maintains a professional demeanor and adheres and promotes professional, ethical and legal standards of nursing.
17. Regular attendance is required.
18. Performs other duties as required.
