You'll collaborate with facility staff, attending providers, and families to optimize treatment plans while maintaining the clinical independence to make evidence-based decisions that improve residents' quality of life. Our providers care for residents experiencing conditions such as: Depression Anxiety Dementia-related behavioral symptoms Grief and adjustment disorders Bipolar disorder Schizophrenia and other serious mental illnesses Responsibilities Conduct psychiatric evaluations and follow-up visits Diagnose and treat psychiatric conditions Prescribe and manage psychotropic medications Review medication regimens in accordance with CMS guidelines Collaborate with interdisciplinary care teams Educate facility staff and families regarding behavioral health conditions Complete timely, compliant documentation Participate in monthly collaborative meetings with supervising psychiatrist Qualifications Board Certified Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) Current Illinois APRN license (or eligibility) DEA registration (or ability to obtain) Previous psychiatric NP experience preferred Long-term care or geriatric psychiatry experience is a plus, but not required About UltaCare Medical Founded in 2006, UltaCare Medical provides high-quality, evidence-based medical and behavioral healthcare to residents of assisted living, skilled nursing, rehabilitation, and long-term care communities.
