- Conduct comprehensive patient evaluations in both inpatient and outpatient environments while crafting tailored treatment plans
- Deliver targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
- Work in tandem with nurses, physicians, case managers, and a variety of rehabilitation specialists to maximize patient outcomes
- Educate patients and their families on therapeutic objectives, discharge planning, and at-home exercise regimens
- Meticulously document assessments, treatments, and patient progress in line with established facility and regulatory standards
- Engage actively in care team meetings and interdisciplinary conferences to maintain coordinated patient care
- Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
- Valid state licensure as a Physical Therapist
- Current CPR certification preferred
- Specialty certifications (for example, neurological or geriatric) are desirable, but not mandatory
- Prior experience in a clinical setting highly preferred
- Proficiency in managing patients with intricate medical and mobility challenges
- Exceptional communication and team collaboration skills
- Familiarity with digital charting and clinical documentation systems required
