Location 7227 E Baseline Rd #126, Mesa, AZ 85209 Compensation $26-$28 / Hour DOE Status Full‑Time, W2 Employee Schedule Monday – Friday, 8 AM – 5 PM About the Role We are seeking a compassionate and dynamic Primary Care Supervisor to join our Atlas Medical Team. A Primary Care Supervisor is responsible for maintaining and growing relationships with existing referral sources and customers such as hospitals, physician offices, case managers, discharge planners, long‑term care facilities, and home health agencies. The focus is on account retention, service quality, and revenue growth within assigned territories. Your key responsibilities will include: Team Supervision & Development Supervise all Atlas departments and employees. Conduct regular one‑on‑one meetings with team members. Review outcomes with the Atlas Practice Manager. Hold team members accountable for workflows and deadlines. Operational Coverage & Training Learn all department roles and workflows. Provide coverage during employee absences. Identify process gaps and support improvements. CCM Program Oversight Manage the Chronic Care Management (CCM) program. Create and maintain care plans. Meet regularly with Sue regarding CCM programs. Communication & Monitoring Monitor facility team threads and ensure timely responses. Collaborate closely with the Atlas Practice Manager. Qualifications To thrive in the role of Primary Care Supervisor you must: Have 2–5+ years in a management/supervisor role. Strong leadership and communication skills. High attention to detail and accountability. Ability to multitask and learn multiple workflows. Possess a valid driver's license, clean driving record, and reliable automobile. Provide proof of current automobile insurance (MVR required). Have a clear negative TB test.
