Job Overview The Social Worker provides a psychosocial perspective to the interdisciplinary evaluation, assessment, care planning, coordination of care, and disenrollment processes of the PACE program. Responsibilities Deliver individual and group interventions, including family contacts, collateral contacts, participant and family education, assessment, counseling, and mobilization of resources. Identify and support behavioral health needs, provide case management and advocacy, and facilitate discharge planning. Apply knowledge of lifespan development, family systems, behavioral health, and social determinants of health to create individualized interventions. Collaborate as a core member of the Interdisciplinary Team to optimize health status and quality of life of PACE participants and their families. Minimum Requirements Master’s Degree in Social Work (MSW) from an accredited university. Minimum of 2 years of experience in case management, social advocacy, and/or mental health with a geriatric population (field work or internship may substitute 1 year). Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred. Valid BLS certification or higher, following AHA or American Red Cross guidelines. Compensation $71,503.24 - $89,379.05 annually. The range is subject to change and may qualify for discretionary bonuses or incentives.
