Licensed Clinical Social Worker (LCSW) - Neuro Community-Based Care We are seeking a Licensed Clinical Social Worker to support individuals and families navigating emotional, behavioral, and life changes that come with neurological conditions. This role focuses on real‑world psychosocial support delivered in home and community environments, helping patients and families adapt, stabilize, and rebuild function in daily life. You will be part of a neuro rehab team that values autonomy, flexibility, and deep clinical impact. $500 sign‑on bonus available for select roles. Job Locations: US‑ME‑Augusta | US‑ME‑Biddeford | Position Type: PRN | Pay: $40.00–$46.00 per hour. Provide therapeutic interventions and resource recommendations that address patient goals to achieve personal, career, and independent living goals in the most integrated setting possible. Sets measurable objectives that are formulated in conjunction with the rehab team. Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems. Instructs and counsels patients and families in treating and coping with social and emotional responses connected with illnesses. Participates in care coordination activities and serves as a resource to other health team members in the identification and resolution of patient needs. Identifies and assists patients and/or families to utilize appropriate community resources to achieve measurable objectives. Qualifications: Master’s Degree in Clinical Social Work (LCSW). Current LCSW licensure in the state of practice. Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis. CPR certification required. Ability to lift 50 pounds. Ability to move, lift, or transfer patients which may involve lifting up to 100 pounds following safety procedures. Fine motor skills, visual acuity, and walking/standing for extended periods. Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times. Salary Range: USD $40.00 – $46.00 / Hour. #J-18808-Ljbffr
