Wellness Nurse (LPN/LVN) - Senior Living

Role Overview The Wellness Nurse (LPN/LVN) supports resident health in senior living settings by conducting wellness assessments, monitoring health changes, and ensuring safe medication practices. They assist with medication management, perform MAR/TAR audits, and maintain accurate documentation to ensure clinical compliance. The role involves communicating with families, physicians, and care teams, monitoring incidents and follow-up care, supporting infection control, and contributing to clinical quality initiatives. Additionally, they train and support care team members to uphold high standards of resident care. Qualifications & Skills Active LPN/LVN license Over 1 year of experience in senior living or clinical settings Strong assessment, documentation, and medication administration skills Excellent communication and problem-solving abilities #J-18808-Ljbffr
