Location: Mt Airy, NC 27030
Facility: LTC/SNF
Shift: 7am - 7pm & 7pm – 7am (Three days in a week that includes EOW)
Contract: 13 weeks
Start: TBD

- Immunization records (PPD, Flu and Physical) if updated
- Covid Vaccination Card
Requirements:
- State LPN license
- BLS/CPR preferred
- Min 1 -2 year of LPN experience in rehab or skilled nursing facility will be preferred.
- Duties include taking vitals, assisting with ADL’s, transportation, medicine administration and reminders, assisting RN’s.
- Care/Rehab or Skilled Nursing Facility is preferred.
