Job Title: Physical Therapist - Home Health
Location: Naples, FL
Pay Rate: $52.50/hr.

Job Type: Direct Hire/ Permanent
Hours: Full-Time 40hrs.

Minimum Qualifications:

• Doctor of Physical Therapy (DPT) from a CAPTE-accredited program
• Current Florida Physical Therapist license
• Current BLS certification through the American Red Cross or American Heart Association
• 1–2 years of physical therapy experience
• Home health experience preferred

Job Summary:
Empath Home Health, a member of Empath Health, is seeking a compassionate and motivated Home Health Physical Therapist (PT) to join its clinical team. The Home Health Physical Therapist (PT) will provide physician-prescribed therapy services designed to help patients achieve and maintain their optimal level of physical functioning, mobility, and independence.

Responsibilities:

• Provide physical therapy services in accordance with physician orders and agency procedures
• Perform initial and ongoing patient assessments, including OASIS assessments at appropriate intervals
• Evaluate mobility, functional abilities, and activities of daily living
• Develop and revise individualized plans of care in collaboration with physicians and care team members
• Establish realistic mobility and functional goals for patients
• Monitor patient response to treatment and progress toward established goals
• Communicate changes in patient condition to physicians and designated agency representatives
• Maintain compliance with applicable federal and state regulations
