Info. & System Security Agreement, Integrity and Audit Services Form, Non Employee Upload Form, Conflict Of Interest Disclosure Form, Criminal Background Check, Education Verification, EPLS, GSA/OIG, NSO, System Access Form, Child and Adult Dependent Abuse Training, IA Sing Report, and OFAC - Trinity Profile required for submission - Trinity Physical/Health Statement needed for onboarding - Trinity PowerPoint Genesis required for onboarding - Influenza Vaccine is optional for onboarding
