Responsibilities Provide on-site rounding care throughout assisted living, memory care, and long-term care communities. Manage pressure injuries, diabetic ulcers, post-surgical wounds, and skin integrity concerns. Support chronic disease management and preventative care initiatives. Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams. Communicate effectively with families, caregivers, and facility staff. Complete documentation, medication management, diagnostics, and lab coordination. Qualifications Active Minnesota Physician Assistant (PA-C) license in good standing. Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred. Strong clinical judgment and communication skills. Valid driver's license and reliable transportation. Commitment to high-quality, patient-centered senior care.
