RN Case Manager, Hospice (Per diem)

Assisted is looking for a per diem RN Case Manager to support our Covina branch covering Whittier and nearby cities and to pick up about 5-7 visits per week. No on‑call requirement for this position! What you will do: Have flexible hours and working close to home (reduce your commute!) Complete initial and on‑going comprehensive assessments of the patient’s needs Provide service and /or treatments requiring substantial and specialized nursing skills Initiate appropriate preventative and rehabilitative nursing procedures Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs Who you are: Graduate of an approved school of professional nursing.
