Bilingual Case Manager — Housing & Advocacy Leader
PATH
PATH is seeking a Bi-Lingual Case Manager to join the Interim Housing Services team at the Evans Lane office in San Jose, CA. This role supports clients through homelessness by coordinating housing, healthcare and social services with a flexible, client-centered approach. You will conduct intake, develop individualized service plans, provide referrals, and maintain confidential records while practicing trauma-informed care and motivational interviewing to help clients achieve housing stability. #J-18808-Ljbffr PATH
