We’re seeking an Care Manager (Registered Nurse) to guide personalized care, support our caregivers, and build trusted relationships with clients and families throughout the Triad. Role Description

The Care Manager (RN) is a full time role based in Greensboro NC, with a combination of in-office and in-home visits. The Care Manager (RN) is responsible for assessing client needs, developing and updating individualized care plans, and coordinating services that support safe, dignified in-home living. Daily activities include conducting wellness assessments, monitoring changes in health status, collaborating with caregivers and families, and communicating with healthcare providers as needed. The role also includes overseeing quality of care, providing clinical guidance to direct care staff, and supporting compliance with standards, policies, and documentation requirements. The Care Manager (RN) works closely with the Comfort Keepers team to ensure care aligns with the principles of Interactive Caregiving™ and elevates the human spirit.

Qualifications

• Ability to perform comprehensive Case Management, including care planning, coordination of services, and ongoing evaluation of client needs.

• Experience in Training direct care staff and caregivers on care protocols, safety procedures, and best practices in senior care.
\n
• Strong background in Elder Care, with familiarity in in-home support, dementia care, fall prevention, and promoting client independence.
\n
• Understanding of Social Services resources and community programs, with the ability to connect clients and families to appropriate support.
\n
• Clinical knowledge of Medicine relevant to geriatric care, including monitoring symptoms, medication awareness, and collaboration with physicians.

• Active Registered Nurse license in North Carolina and good standing with the state board of nursing.
\n
• Excellent communication, documentation, and interpersonal skills, with the ability to work collaboratively in a multidisciplinary setting.

• Comfort with hybrid work arrangements, including in-home visits, office-based tasks, and remote documentation or follow-up.
\n
• Reliable transportation for client visits in the Greensboro / High Point NC and greater Triad Area area and ability to meet any required background checks or credentialing standards.
