• Assess patients in both inpatient and outpatient settings, and craft customized treatment plans.
• Deliver physical therapy interventions that focus on enhancing mobility, strength, balance, and function.
• Collaborate with a diverse team—nurses, physicians, case managers, and rehabilitation professionals—to ensure comprehensive patient care.
• Share knowledge with patients and their families regarding therapeutic goals, discharge planning, and home exercise recommendations.
• Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility guidelines.
• Engage actively in care team meetings and multidisciplinary conferences to foster coordinated care.

To be considered for this role, you'll need:

• A Doctorate in Physical Therapy (DPT) or a relevant equivalent from an accredited institution.
• State licensure as a Physical Therapist, with CPR certification preferred.
• Specialized certifications, such as those in neurological or geriatric therapy, are advantageous but not required.

Successful candidates will possess prior experience, a profound understanding of complex medical and mobility challenges, and exceptional communication skills.
