Overview of the Primary Care Physician Role: The Primary Care Physician will deliver comprehensive primary care services to a panel of veterans, ensuring seamless continuity of care as part of a multidisciplinary healthcare team. Your responsibilities will include assessing and managing both new and returning patients with a variety of medical conditions (such as hypertension, diabetes, ischemic heart disease, COPD, depression, and chronic pain). Care may be provided through in-person visits, telehealth, or video consultations, with occasional travel to VISN medical centers or Community-Based Outreach Clinics (CBOCs) as needed. You will be tasked with ordering diagnostics, consultations, medications, and other therapies as required, addressing preventative health needs, and completing all necessary VA documentation, including clinical reminders and screenings, in a timely manner. Telehealth and video consultations must be conducted from approved VA sites or designated alternate work locations. Additionally, you may assist with the Call Center as required. Key Responsibilities:Veteran -Centered Care: Provide comprehensive primary care services focusing on health maintenance, disease prevention, patient education, and chronic disease management. Encourage shared decision-making with veterans and their families to achieve the best health outcomes. Patient Aligned Care Team (PACT) Model: Implement and adhere to the PACT model, emphasizing collaboration and coordination among the multidisciplinary healthcare team. Apply evidence-based practices to improve patient care and outcomes. Quality Improvement: Engage in ongoing quality improvement initiatives to enhance care delivery. Utilize various quality measurement tools to assess and enhance clinical performance and patient outcomes. Clinical Leadership: Act as a mentor and role model for fellow healthcare providers within the primary care team. Participate in educational activities, offering guidance and support to residents, students, and other trainees. Healthcare Coordination: Coordinate care for veterans, integrating specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery. Collaborate with other healthcare professionals to create individualized treatment plans for veterans.
