Care Coordinator

STATEMENT OF THE JOB: Under the direct supervision of the Manager of Outpatient & Wellness Programs, the Care Coordinator is responsible for providing direct therapeutic and case managements services to children and families in Summitview's Outpatient & Wellness Program. RESPONSIBILITIES: Provide individual rehabilitation and supportive services to youth and caregivers. Provide case management including connecting clients and their families to community resources. Participate and facilitate client and team meetings, including IEPs, CFTs and other required meetings. Provide client crisis response and intervention services as needed. Complete Care Plans and other assessments as needed to help determine treatment goals. Complete all documentation within requirements as noted in policies and procedures. Meet the agency's productivity requirements for billable services. Work effectively as part of a multi-disciplinary and inter-agency team, coordinating with therapists, psychiatrists, case managers, residential staff, and teachers, in addition to social service agencies, healthcare providers and other community partners and stakeholders. Provide services in a variety of settings, including the office, in the home, school, community and virtually. Use a personal vehicle to transport clients and family members when necessary. Other duties as assigned. JOB EXPECTATIONS: Adheres to all policies and procedures including, but not limited to, HIPAA, PHI, Mandated Reporter Status & Procedures, Professional Relationships with Youth & Non-Minor Dependents, and all other Federal and State laws. Follows agency policy for crisis intervention and management skills. Attends scheduled in-service training programs. Attends weekly team meetings. Completes assigned program operational tasks in accordance with supervisory directives. Displays sensitivity to the service population's cultural and socioeconomic characteristics. Demonstrates effective oral and written communication skills. Works cooperatively with others in a team-oriented manner. Must participate in Performance Quality Improvement and Risk Prevention. Provides and practices trauma-informed care services and methods; actively uses QUALIFICATIONS: Required Must be at least 21 years old. Associate degree in behavioral health or a related field (e.g., social work, psychology, sociology, social welfare, etc.). One or more years of experience providing individual rehabilitation and skill building services to youth with complex trauma and mental health needs and their caregivers. One or more years of experience providing case management and care coordination services. Preferred Bachelor's degree in behavioral health or a related field (e.g., social work, psychology, sociology, social welfare, etc.). Knowledge of trauma informed care principles, DBT, Motivational Interviewing, and other evidence-based practices. Knowledge of El Dorado County community resources and systems. Knowledge of insurance, particularly navigating the Medi-Cal system. ELIGIBILITY REQUIREMENTS: Must possess a valid, unrestricted California Driver's License; personal automobile liability insurance; and a driving record acceptable to Summitview's insurance carrier. Must be physically and mentally fit; must be willing to complete a health screening including physical examination as required by our state licensing agency Community Care Licensing (CCL); and must have the unrestricted capacity to employ physical restraint techniques in a professionally safe manner. Must be willing to present a recent negative Tuberculosis test or be willing to take one. The COVID-19 vaccination inclusive of booster doses when eligible is strongly recommended but not required. Must be willing to complete and pass a Department of Justice (DOJ) personal background investigation to meet the requirements of our state licensing agency Community Care Licensing (CCL). OTHER: Employment is at-will. This position is an hourly, full-time, non-exempt position. Standard work hours are Monday-Friday with flexibility to meet client scheduling needs.
