Your future role at a glance Location: Indianapolis, IN Facility: Ascension St. Vincent Department/Specialty: Cardiac Rehab Schedule: Days | Full time; Monday - Friday with rotating Saturdays every 6 weeks. A typical schedule is 7:00-3:30; shift time can be flexible in this role. No Sundays and no major holidays. Assess patient readiness for activity advancement and safely implement, monitor, and adjust cardiac rehabilitation and mobility plans in collaboration with the care team. Coordinate discharge planning needs with multidisciplinary team members to promote safe transitions of care and optimize patient outcomes. Monitor, document, and communicate patient condition, response to treatment, and changes in clinical status in a timely and accurate manner. Develop and maintain patient-centered care plans while promoting quality, safety, and evidence-based practice throughout the continuum of care. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required. What additional preferences we're seeking Inpatient cardiac nursing experience preferred.
