Registered Dietitian (RD) – Long-Term Care

Position Summary

Spring River Rehabilitation & Care Center is seeking a Registered Dietitian (RD) to join our interdisciplinary team. This role is responsible for overseeing the nutritional care of residents in a skilled nursing and long-term care setting, ensuring compliance with regulatory standards while supporting resident-centered care.

Key Responsibilities

• Complete nutrition assessments for all new admissions, readmissions, and significant changes

• Develop and update individualized nutrition care plans

• Monitor weight trends, intake, and nutritional status

• Provide recommendations and interventions for:

  • Weight loss/gain

  • Wound healing

  • Malnutrition

  • Specialized diets

• Collaborate with:

  • Nursing

  • Providers

  • Dietary staff

  • Interdisciplinary team (IDT)

• Participate in:

  • Care conferences

  • QAPI initiatives

  • Survey readiness and compliance activities

• Ensure compliance with:

  • CMS regulations

  • State guidelines

  • Facility policies

• Review and support:

  • Menu compliance

  • Therapeutic diet accuracy

  • Food service practices

Qualifications

• Registered Dietitian (RD/RDN) credential required

• Licensed in the State of New Mexico (or eligible)

• Experience in long-term care or skilled nursing preferred

• Strong understanding of:

  • CMS regulations

  • Nutrition support in elderly population

  • Wound care nutrition

• Excellent communication and documentation skills
