Registered Nurse Care Coordinator

Job TitleHelpDutiesThis is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until Sept 30, 2026. The initial cut-off date for referral of eligible applications will be July 27, 2026. Eligible applications received after that date will be referred at regular intervals on an as-needed basis until positions are filled. All referred applicants receive a final notification once a selection decision has been made.Major duties include but are not limited to:Coordinate timely, Veteran-centered care for Veterans receiving services through VA and community care pathways.Triage requests for services and help direct Veterans to appropriate care options based on clinical needs and established processes.Develop and support care coordination plans that promote continuity, access, and successful completion of the episode of care.Review medical documentation from community providers to support informed care coordination and follow-up.Serve as a liaison between Veterans, VA care teams, and community providers throughout the consult and referral process.Guide Veterans through available care options, including direct VA care and care in the community.Collaborate with interdisciplinary teams to support timely referrals, scheduling, communication, and care outcomes.Document, exchange, and track care coordination information using VA health information systems and required care coordination processes.Pay: Competitive salaryLicensure: 1 full and unrestricted license from any US State or territoryWork Schedule: This is an intermittent position. Intermittent employment is defined as non-fulltime employment in which employees are appointed without a regularly scheduled tour of duty.
