Work Environment\n

As a telehealth-focused facility, we offer flexibility in your work environment. You will conduct virtual counseling sessions with clients from the comfort of your home, utilizing advanced telecommunication technologies. Key Responsibilities\n

• Conduct comprehensive evaluations to assess the ongoing needs and progress of patients receiving care.
\n
• Facilitate effective communication among multidisciplinary team members, including supervisors, direct care staff, family members, and other stakeholders to support patient development.
\n
• Create detailed plans of action outlining appointments, home visits, and observation sessions to actively monitor client progress.
\n
• Organize and execute home visits for in-home assessments, including data collection and client reviews.
\n
• Ensure the implementation and fidelity of therapeutic programs, tailoring interventions to meet individual client needs.
\n
• Participate in interdisciplinary meetings and contribute to discussions about patient care and treatment modalities.
\n
• Document monthly and quarterly progress notes, summarizing patient advancements and changes in treatment strategies.
\n
• Facilitate training sessions to equip colleagues and team members with the resources needed to carry out our program plans effectively.
\n
• Conduct follow-up engagements through interviews and home visits to ensure continuity of care and address evolving client needs.
\n
• Maintain organized, up-to-date records to keep relevant parties informed about client progress and treatment strategies.
\n
• Act as a liaison between patients and community resources to enhance connection and support networks.
\n
• Contribute to developing training materials aimed at improving individual and group therapy programs.

Patient Population / Client Focus\n

In this role, you will have the unique opportunity to work with a diverse range of client populations, including both adults and youth dealing with various challenges such as anxiety, depression, trauma, family dynamics, and substance misuse. Your expertise will empower clients to navigate their mental health journeys through tailored therapy sessions.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active, unrestricted license as a Professional Counselor (LPC) within the state of Oregon.
\n
• A minimum of two years of counseling experience in clinical settings is preferred.

• Demonstrated specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.

• Solid understanding of legal, ethical, and confidentiality standards related to counseling practices.
\n
• Exceptional interpersonal and communication skills, with the ability to establish rapport across diverse clientele.

• A proven capacity to collaborate effectively within a healthcare team environment.

• Proficiency in using electronic medical records (EMR) software and other digital tools relevant for telehealth practices.

Preferred Experience\n

Compensation and Benefits\n

We are pleased to offer a competitive salary ranging from USD 80,000 to 100,000 annually , commensurate with experience and caseload. Additional benefits include:
