RN - Hospice/Home Health

Pride Health is hiring an RN - Hospice/Home Health to support our client's healthcare facility in Fayetteville, NC.

Job Details:

• Schedule: Monday-Friday | 8:00 AM-4:30 PM | On-Call/Weekend: Yes, approximately 1-2 weekday and 1-2 weekend on-call shifts
• Location: Fayetteville, NC; Coverage areas include a 50-mile radius from the office.
• Mileage reimbursed: Yes
• Job Type: Contract- 9 Week/s
• Pay Range: $52-$54

Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Manage and coordinate hospice patient care as an RN Case Manager.
• Conduct patient admissions and coordinate care services.
• Complete accurate and timely patient documentation and follow-up.

Requirements:

License(s): Active RN license Certification(s) : BLS/CPR Years of experience: 1+ Skills/Specialties/Technology: Hospice experience required; PointCare experience preferred
