- Physical Statement (must be dated within 9 month of hire), with habituation statement.
- Negative PPD Information Step 1 and Step 2 (within 1-3 weeks from each other) OR Whole Assay Blood Test.
- Positive PPD (needs the Following): A) Written documentation by Doctor with history of Positive PPD B) Negative Chest X-Ray Report.
- Rubella Immunity Proof (lab Printed Report).
- Rubeola Immunity Proof of (Lab Printed Report).
- Drug Screen (Lab Printed Report).
- Proof of Flu Vaccine for the season (if you did get it).
- Direct Deposit form, Voided check or Routing & Bank account numbers.
