Job Overview The Patient Navigator is a Licensed Vocational Nurse (LVN) who actively assists physicians and the healthcare team in the day‑to‑day activities and care of patients. In this leadership role, the Navigator coordinates patient care within the unit’s service line, rounds with colleagues, identifies patient needs, and collaborates with physicians, RN coordinators, preceptors, and other staff to facilitate education, training, and smooth care transitions. Responsibilities Assist physicians in daily patient care activities and care coordination. Arrange and oversee details of patient care within the unit’s service line. Lead patient rounding and care activities, identifying and assessing patient needs. Solve scheduling and coordination issues with physicians, RN coordinators, employees, and preceptors for new employee orientation and training. Collaborate with RNs and MDs to prepare, teach, and guide patients through the continuum of care. Execute patient care duties, support system processes, conduct research, provide education, and demonstrate professional leadership. Initiate and coordinate patient teaching regarding care plans and follow‑up under provider and clinic leadership. Review all medications and perform medication reconciliation. Communicate with the medical team and treating/attending physicians regarding patient status. Under the supervision of the Manager and attending physician, oversee patient flow through the continuum of care, including test scheduling and the admissions process. Coordinate consults with physicians and relevant services such as social services, nutritional services, case management, transportation, radiology, and laboratory. View, follow, and evaluate laboratory results. Serve as the primary contact and communication liaison between patients, the health‑care team, and community partners. Provide patient and family education—including disease process, health‑care system navigation, referral management, pre‑ and post‑operative care, and expectations. Build and maintain relationships with external vendors, relaying health‑education and training to faculty and staff. Perform additional duties as assigned. Collaborate with the RN Coordinator to facilitate the monthly Bladder Cancer Support Group with Social Services. Required Qualifications High school diploma or equivalent. Specialized or technical training. Graduate of an accredited Vocational Nursing Program. Minimum of 2 years of experience in acute or ambulatory care as a Patient Navigator or LVN. Excellent communication skills and proficiency in speaking, reading, and writing English. Strong time‑management, organizational, interpersonal, and research skills. Ability to negotiate and demonstrate excellent customer‑service behavior. Capability to function independently and collaboratively within a team. Proficiency in Word processing, Microsoft Outlook, PowerPoint, Excel, electronic health records, and related utilization systems. Knowledge of medical terminology. Preferred Qualifications 1–2 years of experience in case management. Required Licenses and Certifications Licensed Vocational Nurse – LVN (CA DCA). Basic Life Support (BLS) certification from the American Heart Association. Fire Life Safety Training (LA City).
