TCM Care Manager ExtenderDepartment: Tailored Care ManagementLocation: Hybrid (In-Office, Field, and Remote)Service Area: Cleveland CountySchedule: Monday – Friday | 8:30 AM – 5:00 PMResponsibilitiesPerforming general outreach, engagement, and follow-up with members.Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation).Conducting introductory engagement calls to enroll members into TCM services.Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing.Sharing information with the care manager and other members of the care team on the member’s circumstances.Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.Support the care manager in assessing and addressing unmet health-related resource needs to include following up on ADT alerts and other urgent TCM needs.Other duties as assigned.Qualifications18 years of age.Has a high school diploma or equivalent.Be a person with lived experience with a behavioral health condition and a Certified Peer Support SpecialistHas one year of paid experience performing the types of functions described in the “Extender Functions” section below.Individuals who will be able to meet these qualifications, including, but not limited to the following:Certified Peer Support Specialists; orCommunity health workers (CHW), defined as individuals who have completed the NCCommunity Health Worker Standardized Core Competency Training (NC CHW SCCT); orIndividuals who served as Community Navigators prior to the implementation of Tailored PlansKnowledge, Skills, and Abilities:Person-Centered Thinking/planningKnowledge of using assessments to develop plans of care.Knowledge of LOC processesKnowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plansFamiliarity with or intent to learn the use of Motivational Interviewing techniques.Strong interpersonal and written/verbal communication skills.Conflict management and resolution skillsProficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance.Ability to make prompt, independent decisions based upon relevant facts.Good organizational skills to prioritize duties and work with minimal levels of onsite supervision to consistently meet deadlines.We invite qualified candidates who are passionate about patient care to apply for this rewarding opportunity as a Care Manager Extender.About Epiphany Family ServicesEpiphany Family Services is committed to providing an integrated system of care to individuals from diverse backgrounds.
