Registered Nurse - Hospice

Hospice Registered Nurse Opportunity

Seeking experienced Hospice Registered Nurses to provide compassionate in-home and facility-based care to terminally ill patients across multiple counties. The role involves managing end-of-life care in various settings, ensuring comfort and dignity for patients and their families.

Responsibilities:

• Administer and coordinate hospice care plans for terminally ill patients in-home and nursing facilities.
• Collaborate with a multidisciplinary team to develop and implement patient-specific care strategies.
• Conduct 5-6 patient visits daily within an 8-hour shift, providing clinical assessments and symptom management.
• Document patient care accurately using the Epic EMR system and communicate updates effectively to the team.
• Monitor patient comfort and adjust treatments according to changing needs and protocols.
• Educate patients and families about disease progression, medication management, and available support resources.

Qualifications:

• Hold a valid WV Registered Nurse license.
• Possess a minimum of 1 year of hospice care experience.
• Certification in BLS is required.
• Demonstrate proficiency with Epic EMR system.
• Possess strong communication and compassionate patient care skills.
• Ability to work effectively with a small team covering multiple counties.
