Physical Therapist - Home Health

Physical Therapist Home Health Provides therapeutic treatment to homebound patients to alleviate pain, restore function, and maintain optimal performance using various modalities. Documents treatments accurately in electronic medical records and reports significant patient condition changes to the physician. Responsibilities: Promotes patient and family education tailored to cultural and personal needs, collaborates on self-care techniques, and ensures timely documentation per agency policies. Maintains compliance with federal, state, and agency regulations, including incident reporting and regulatory standards. Participates in mandated activities such as in-services, Quality Assurance, and staff meetings. Qualifications & Skills: Must hold an unencumbered State of Michigan Physical Therapy license, with DPT preferred. Minimum one year of home care experience, competent in assessment and documentation, with knowledge of OASIS-C preferred. Demonstrates sound clinical judgment, excellent communication skills, and proficiency in clinical skills.
