Responsive RecruiterReports To: Agency Owner/AdministratorJob Summary: Demonstrate proficient nursing skills utilizing assessments for admission, transfer, reassessment, and discharge based on assessed needs. Effectively utilize the nursing process to assess, diagnose, prioritize problems, treat, and evaluate. Possess excellent clinical skills and judgment. Ability to work in an independent practice environment. Coordinate and supervise caregivers and licensed personnel in the delivery of consumer care. Maintain compliance with Agency policies, procedures, and federal, state, and local regulations.Qualifications/Educational Requirements:Registered Nurse (RN)Possess a current, unencumbered license to practice nursing within the StateCurrent CPR certificationMinimum of one (1) year of acute care nursing experience as a RNExcellent oral and verbal communication skillsWork collaboratively with consumers, families, and staffOne (1) year of Home Health Care experience preferredResponsibilities/Essential Functions:Perform the initial consumer assessment and reassessment, as needed, with input from the consumerEducate, coach and monitor consumers and caregivers on disease process, medications, treatments, and medical/nursing plan of care plan and changes to the plan to ensure consumer's needs are metPrepare clinical notes; submits paperwork in a timely manner per Agency policyDelegate tasks to caregivers and licensed staff according to established standards, skill level required, and ability to perform task to achieve desired outcomePerform supervisory visits and monitor outcomes of delegation to ensure all direct care staff are adhering to the plan of care to meet the needs of the consumerAdminister medications and treatments as prescribed by the consumer's physician or legally authorized practitioner and/or delegate as allowedAttend in-service programs, continuing education, seminars, and self-study programs annually to maintain licensure and professional skill setMaintain current CPR certificationPerformance Standards:Adhere to HIPPA and maintains consumer confidentialityReport signs of abuse and neglect, mistreatment and misappropriation or exploitationDemonstrate and understand the Consumer Rights and ResponsibilitiesObserve, report, and document consumer status and care provisions in a timely manner and in accordance with Agency policyProvide care in compliance with Agency's established protocols in a competent, safe, and timely mannerApply safety concepts in the work place and in accordance with Agency policiesMaintain a clean and neat work environmentProvide emotional support and motivation through conversation, encouragement, and recreational activities to meet consumer/family needsAdhere to infection control and safety procedures that are in accordance with Agency policiesDemonstrate a positive and professional manner according to Agency policyAddress and treats co-workers respectfully to maintain a professional work environmentAttire and appearance is appropriate and adheres to Agency policies and proceduresMaintain acceptable attendance status per Agency policyDemonstrate effective time management skills with daily assignmentsDocument activities legibly and accurately on required Agency recordsSubmit paperwork in a timely manner per Agency policyAttend all mandatory staff meetings, annual training, and additional In-Services if necessaryAccept assignments willingly and perform other duties as assignedPhysical Elements:Sufficient clarity of speech and hearing or other communication capabilities, with or without reasonable accommodation, to enable the employee to communicate effectively; Sufficient vision or other powers of observation, with or without reasonable accommodation, to enable the employee to review a wide variety of materials in electronic or hard copy form; Sufficient manual dexterity, with or without reasonable accommodation, to enable the employee to operate a personal computer, telephone, and other related equipment; and Sufficient personal mobility and physical reflexes, with or without reasonable accommodation, to enable the employee to safely lift, move, or maneuver whatever may be necessary to successfully perform the duties of their position; function in a general office and frequent travel to a variety of field sites.
