RN Case Manager: Discharge & Care Coordination

St. Charles Health System in Bend, Oregon is seeking an RN Case Manager. This position is responsible for providing case management to ensure effective patient care and discharge planning. You will collaborate with healthcare teams to create and implement discharge plans and document patient progress accordingly. The ideal candidate will have at least 3 years of clinical nursing experience, an Oregon RN License, and a commitment to quality improvement.
