Community Support Facilitator

Life Steps Southern California Adult Services (LSF SoCal) is a nonprofit organization serving more than 300 adults with developmental and physical disabilities throughout Southern California. Through individualized, one-to-one services, LSF SoCal supports independence, personal choice, and each person’s desired quality of life. As a Community Support Facilitator working in and around Huntington Beach and Buena Park, you will provide home-based services to adults with developmental disabilities, helping them build everyday living skills, access community resources, and work toward person-centered goals.

Responsibilities

• Develop person-centered plans that reflect each individual’s needs, choices, and goals.
• Identify natural and community supports and help individuals access and use them.
• Teach life management skills related to paying bills, transportation, and housing.
• Provide mobility training, including teaching adults with developmental disabilities to use public transportation.
• Provide physical assistance when needed.
• Assist individuals in creating health and safety plans.
• Submit accurate, complete billing records and service documentation.
• Prepare and submit complete, accurate reports to Regional Center caseworkers.
• Follow the monthly schedule and participate in required trainings and meetings.

Qualifications

• High school diploma or GED.
• Reliable transportation, a valid driver’s license, and auto insurance.
• Good oral and written communication skills.
• Prior case management experience preferred.
