CNA - LTC/SNF

Job Description

Job Summary:

Seeking compassionate and detail-oriented CNA to provide quality care to patients in long-term care or skilled nursing facilities. The ideal candidate is committed to maintaining patient dignity and comfort while assisting with daily activities.

Responsibilities:

• Assist patients with activities of daily living including bathing, dressing, and mobility.
• Monitor and report changes in patient condition to healthcare team.
• Follow facility protocols for infection control and safety procedures.
• Maintain accurate documentation of patient care activities.
• Support patients with feeding and personal hygiene tasks.
• Adhere to facility policies, including ID badge and credential requirements.

Qualifications:

• Current CNA certification valid in the state.
• Proven experience in LTC or SNF settings preferred.
• Ability to work rotating shifts including weekends.
• Demonstrate strong communication and teamwork skills.
• Compliance with COVID-19 vaccination or approved exemption documentation.
• Ability to lift, move, and assist patients with mobility.
