Palliative Oncology Nurse Navigator

YOUR ROLE As a Thyme Care Palliative Care Nurse Navigator, you will serve as a clinical resource for members with serious illness and their care network. Your responsibilities include conducting conversations about goals, values, advance care planning and palliative care via phone, email, or video; completing comprehensive symptom assessments; supporting members longitudinally; facilitating end‑of‑life care discussions when appropriate; and addressing clinical issues while managing escalations. Shift: 11:30 AM – 8 PM ET with optional Saturday coverage as needed. RESPONSIBILITIES Familiarize yourself with Thyme Care systems, tools, technology, and partners, conducting a minimum of 20 member calls per day. Collaborate closely with Nurse leaders and Medical Directors to ensure alignment with clinical protocols and best practices. Establish trusting relationships with members and their care network, prioritizing empathy and active listening. Adhere to Care Team policies, procedures, and documentation standards, contributing to efficient operations and maintaining quality standards. Support members with serious illness and at the end of life, coordinating care and providing clinical support as needed. Participate in case conferences to monitor member progress, provide updates, and collaborate on targeted support plans with the healthcare team. Foster strong partnerships with payers and providers to optimize care delivery and minimize readmissions. Collaborate with non‑clinical Care Team members to address social determinants of health needs, such as food resources and transportation access. Be available for urgent clinical escalations and provide clinical consult support as required. Perform other projects and duties as assigned and as related to department business needs and objectives. QUALIFICATIONS An active, unrestricted compact Registered Nurse (RN) license is required. Licensure in New York, Illinois or Michigan is preferred. Degree in Nursing (ADN or BSN) required; BSN preferred. Minimum five years of nursing experience, including at least three years in outpatient palliative care and/or home hospice caring for oncology patients and their caregivers. Certifications are preferred: Certified Hospice and Palliative Nurse (CHPN) or oncology‑related designations such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM). OCN certification must be obtained at hire or within two years. Fluency in Spanish, written and verbal, is preferred. ADDITIONAL INFORMATION Pay rate: $45.67 per hour (non‑negotiable). Full‑time, non‑exempt position located within the lower 48 United States. Optional up to three hours of Saturday coverage may be required.
