Summary

Case Manager RN opportunity supporting patients and families throughout their healthcare journey in an acute care hospital setting. This role focuses on care coordination, discharge planning, patient advocacy, and collaboration with interdisciplinary teams to ensure safe and effective transitions of care. Ideal for experienced acute care Case Management RNs seeking a weekday schedule.

Requirements

• Active RN or APRN License
• Associate Degree in Nursing or RN Diploma
• Minimum 2 years of recent acute care Case Management RN experience (within the last year)
• Strong discharge planning and care coordination experience
• Experience working across multiple inpatient units

Pay

• Weekend Differential: +$1.50/hour
• Sign-On Bonus: Case-by-case
• Relocation Assistance: Case-by-case

Shift

• Full-Time Days
• Monday–Friday
• Every other Saturday (potentially every third Saturday once fully staffed)
• Shift options:
• • 8:00 AM – 4:30 PM
  • 9:00 AM – 5:30 PM
  • 10:00 AM – 6:30 PM
  • 11:00 AM – 7:30 PM
