Cooperate closely with nurses, physicians, case managers, and multidisciplinary rehab teams to maximize patient outcomes. Guide patients and their families in understanding therapeutic objectives, discharge processes, and at-home exercise routines. Maintain accurate documentation of patient evaluations, treatments implemented, and progress made in compliance with facility and regulatory standards. Engage in care team meetings and interdisciplinary discussions to assure coordinated patient care.

Doctoral degree in Physical Therapy (DPT) or an equivalent qualification from an accredited educational institution. Valid state licensure as a Physical Therapist. CPR certification is preferred. Additional specialized certifications (e.g., in neurology, geriatrics) are beneficial but not mandatory.

Prior experience is highly desired. Competence in treating patients with a diverse range of medical and mobility challenges. Exceptional communication and teamwork abilities. Familiarity with digital charting and documentation software.
