Schedule/Hours:

• Primarily outpatient care
• Floating to acute care as needed

• Valid and current Physical Therapist license
• Experience in outpatient and acute care settings a plus
• Strong problem-solving, organizational, and communication skills
• Able to delegate and supervise Physical Therapist Assistants and Aides
• Comfortable with Microsoft Windows and ongoing technology training
• Demonstrated commitment to patient safety and confidentiality

• Assess patient function using formal and informal methods
• Develop individualized treatment plans in coordination with physicians
• Supervise and delegate tasks to PTAs and Aides
• Educate patients, families, and caregivers on therapies and home routines
• Document responses to treatment and any status changes per policy
• Contribute to process improvement, program development, and community education
• Participate in interdisciplinary team meetings and conferences
