LPN Visit Nurse

Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes. Under supervision, you will deliver high-quality clinical services, document patient visits thoroughly, and participate in care coordination activities to ensure optimal patient outcomes. Key Responsibilities Provide nursing care within the scope of practice, following the care plan and infection control procedures. Accurately document patient visits in electronic medical records and visit verification systems. Report patient needs and changes in condition promptly, participate in case conferences, and communicate effectively with patients, families, and healthcare team members. Qualifications & Skills Current, unrestricted Licensed Practical Nurse licensure in the practicing state. CPR certification, valid driver’s license, and reliable transportation. Ability to work independently in diverse home environments regardless of patient background. Preferred Attributes Home care experience, strong communication and organizational skills, and flexibility to meet patient needs. Ability to manage multiple tasks and work independently.
