Social Worker II– Master's Level (Full-Time – 35 Hours/Week)

Shift Schedule: Monday–Friday, 9:00 AM – 5:00 PM

As a Social Worker II, you will provide expert clinical case management and crisis intervention services to older adults. Utilizing your advanced training, you will conduct in-depth assessments, develop complex care plans, and advocate for essential government benefits to help mentally and physically impaired seniors maintain their autonomy and quality of life.

What You'll Be Doing

Responsible for the clinical oversight of a diverse caseload, you will ensure that every client receives a personalized, holistic approach to care.

Key Responsibilities:

• Clinical Assessment & Triage: Perform preliminary screenings and emergency triage; conduct comprehensive In-Home and In-Office assessments using standardized tools (e.g., PHQ-9, NSI, STARS).
• Holistic Case Planning: Develop and implement written case plans that include individual and family counseling to resolve complex social and emotional challenges.
• Direct Service Coordination: Arrange for critical supports such as home-delivered meals, home care, Medicare/Medicaid, SNAP, SCRIE, and psychiatric therapy.
• Financial & Legal Protection: Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent exploitation.
• Field Monitoring: Conduct ongoing in-home visits to evaluate functioning, monitor safety risks, and update care plans in response to changing needs.
• Community Advocacy: Represent and interpret JASA's mission at community conferences and meetings; maintain cooperative relationships with partner agencies.
• Administration: Maintain meticulous case records, reports, and statistics within mandated timeframes utilizing program-specific software.

What You'll Need

• Education: Graduate of an accredited college or university with a Master of Social Work (MSW) or equivalent Master's-level degree (Counseling, Gerontology, Psychology).
• Licensure: New York State Certification in Social Work preferred; LMSW strongly preferred.
• Technical Skills: Computer literacy with the ability to navigate electronic record systems; strong writing and record-keeping skills.
• Communication: Proficiency in English required; ability to provide empathetic and clear guidance to clients and families.
