Salary: $110,000 - $150,000 per year (DOE)
We are seeking a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) to provide comprehensive primary care services in an outpatient setting. This role involves delivering high-quality, patient-centered care through assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions.
The ideal provider practices independently while collaborating with a multidisciplinary team and following established clinical standards and protocols.
Key Responsibilities- Provide primary care services to patients across the lifespan (FNP) or adult/geriatric populations (AGNP)
- Assess, diagnose, and manage acute and chronic medical conditions
- Deliver preventive care, health screenings, and patient education
- Develop and implement evidence-based treatment plans
- Prescribe medications and order diagnostic tests as appropriate
- Maintain accurate and timely clinical documentation in the EHR
- Coordinate referrals and collaborate with other healthcare professionals
- Ensure compliance with clinical, regulatory, and safety standards
- Graduate of an accredited Nurse Practitioner program
- Certified as an FNP or AGNP
- Active Arizona RN and APRN licenses
- Current, unrestricted DEA registration
- BLS / CPR & AED certification (AHA or Red Cross)
- Ability to manage acute and chronic conditions independently
- Proficiency with electronic health records
- Strong communication and interpersonal skills
- Cultural competence and commitment to serving diverse populations
- Valid Arizona driver's license
- Level One Fingerprint Clearance Card (required for certain locations)
